# Lost driving licence in Ireland: prepare your NDLS replacement documents

Apply to NDLS for a replacement Irish driving licence online with a Public Services Card and verified MyGovID, or book an NDLS centre appointment. Prepare the completed Garda declaration and the supporting documents for your route; a saved photograph does not replace the licence.

If your Irish driving licence is lost, stolen or damaged, start with the replacement process for that licence. An old photo can help you read its details, but it does not complete the declaration or supply a replacement card. This guide covers replacing an Irish licence, rather than exchanging a foreign one.

## Choose your NDLS route

| Your situation | What to prepare first |
|---|---|
| You have a Public Services Card and verified MyGovID | Online replacement application and required attachments |
| You need to apply in person | Book an NDLS centre appointment and gather its evidence |
| Your address or medical details have changed | Check the additional evidence before applying |
| You find the old licence after applying | Do not start using it again |

The [NDLS replacement page](https://www.ndls.ie/licensed-driver/replace-my-lost-stolen-driving-licence.html) lists the requirements for both routes. Use its application link directly and keep your login credentials separate from document copies.

Keep personal document copies and your replacement paperwork together in Dokus, then update your saved licence copy when the new card arrives. 4.8 out of 5 stars [Free on the App Store](https://apps.apple.com/ie/app/dokus-private-id-wallet/id6781915709)

## Complete the Garda declaration

Download the [Garda declaration linked by NDLS](https://www.ndls.ie/images/Documents/170552_RSA_NDLS_Garda_Declaration_Form_Final_for_FO_2021.pdf). The form asks for your name, address, date of birth and PPSN, and whether the licence was lost, stolen or damaged. Complete it in block capitals and have the declaration witnessed and stamped at your local Garda station.

Keep the completed version distinct from the blank download. Before uploading or bringing it to an appointment, check that the selected reason, signature, Garda details and station stamp are present and readable. The form also says that an old licence found after a replacement application is no longer valid.

## Gather only the supporting evidence your route requires

For the online route, NDLS requires a Public Services Card and verified MyGovID, the completed declaration and the application fee. Its checklist also calls for recent address evidence in the specified address-change circumstances, and a medical report if medical details have changed since the previous application. Check those conditions against your own record rather than uploading every document you hold.

For an appointment, start from the replacement checklist and review the [NDLS in-person document list](https://www.ndls.ie/how-to-apply/applying-in-person.html). It identifies acceptable photo ID, PPSN evidence, address evidence and residence evidence where needed. Photocopies of photo ID are not accepted as primary identity evidence.

When address evidence is needed, NDLS says it must be no more than six months old and match the application address exactly. Its examples include qualifying bank correspondence and utility bills; mobile phone bills are excluded. Open the actual document and check its date, full name and complete address before relying on it.

## Organise the application in four steps

1. Make a short list for your chosen route: declaration, identity evidence, any conditional address or medical evidence, and payment.
2. Keep electronic originals unchanged where available. If a file contains a digital signature or verification code, do not replace it with a screenshot or altered PDF.
3. For an appointment, assemble the physical evidence requested. A document visible on your phone does not automatically meet a requirement to present an original.
4. After submitting, save the confirmation and reference. If you need to follow up, start with that reference rather than sending your full identity bundle again.

Check the current replacement fee and payment arrangements on the NDLS page before applying. Appointment availability and delivery dates can change, so do not build a travel or work deadline around an assumed turnaround.

## Update your copy after replacement

When the replacement card arrives, check its details before making a fresh personal copy. Clearly mark any old scan you retain as superseded, and verify the current file before sharing. Ask a car-hire company or other recipient about its own document requirements; a stored scan is not a guarantee of acceptance.

## Final document check

- The correct NDLS application route is selected.
- The Garda declaration is completed, witnessed and stamped.
- Required evidence is current and its details match.
- Physical documents are ready for an appointment where needed.
- Old licence copies will be replaced or clearly labelled.

For a separate document copy your recipient accepts, the [Irish document tools](/en-ie/tools/) prepare IRP, passport and driving licence copies. These tools do not certify the document; use the original whenever an unchanged file is required.

## Questions about Irish licence replacement

### Can I apply online without verified MyGovID?

NDLS lists a Public Services Card and verified MyGovID as requirements for its online replacement route. If you cannot use that route, check the appointment process and required evidence.

### Can I use the old card if I find it later?

The Garda declaration says that a licence found after applying for a replacement is no longer valid. Do not treat the recovered card as the current licence.

### Is a saved photo enough for the NDLS appointment?

No. NDLS does not accept photocopies of photo ID as primary identity evidence. Follow the official document list for your appointment rather than relying on a personal scan.
