Use the form for the right kind of record
Bermuda Hospitals Board’s Subject Access Request Form lists passport or driver’s licence as identity evidence. It expressly sends requests for medical records to HIMS or the MWI release process instead.
A person making a request for somebody else may need additional identity and authority evidence. Keep each person’s required identity and authority evidence distinct, and follow the form’s instructions. Combine reference files only in an arrangement the recipient accepts. These copy tools do not sign an authority letter, retrieve hospital records or submit the request.
Prepare the requested identity files
Use Letter for a convenient starting layout, or change to A4 if requested. The driver’s-licence preset uses a full-page layout so it does not assert a physical card size. Only the Passport preset uses a standard passport-page size.
The 1 MB compression target is a tool default. No universal BHB email ceiling is claimed. Agree any watermark or redaction before preparing the reference copy.
Files stay in your browser
Choose JPG, PNG, browser-supported HEIC or PDF up to 25 MB per file. Photo to PDF, Compress, Watermark and Hide details process only the first ten input PDF pages and export JPG or an image-based PDF. Those editing copies lose selectable text, fillable fields and digital-signature validation. Keep the original signed or fillable files.
Merge PDF documents joins 2 to 20 files, up to 100 MB altogether, into one PDF. It includes every input PDF page and retains existing selectable text, within file and device-memory limits. Each photo fits on a separate Letter page with 12 mm margins. Its controls order whole files, and its result does not preserve digital-signature validation. PDFs requiring an opening password cannot be added.
All five tools process the selected documents in your browser, without an account or document upload. They leave your source files unchanged and add the free-tool credit to the downloaded copy. The four image-editing jobs remove camera metadata from their exports.

Is this the BHB medical-records request process?
No. BHB’s personal-information form directs medical-record requests to HIMS or the MWI release process. Use the correct official form for the record you need.
Does a prepared PDF prove my authority to act for someone?
No. Follow the recipient’s separate identity and signed-authority requirements. These tools only prepare an attachment.
